PPP Pre-Application Checklist

In order to process your application, Greylock requires the following documents:

  • Your completed and saved application along with the Greylock Supplemental Form
  • Schedule of employees
  • A worksheet/spreadsheet in any format you like showing the calculations and date range used to determine your average monthly payroll costs with supporting documents (e.g. W-3, 941, third party payroll processor records or tax returns) Here is a basic form you can use.
  • 2019 Fed Business Taxes or 2019 Profit/Loss Statement
    NOTE: Sole proprietor must provide a complete copy of your 2019 1040, a 2019 SCH C (https://www.irs.gov/pub/irs-pdf/f1040sc.pdf) with all schedules attached. Please submit proof your business was operational on February 15, 2020. This could be in the form of a bill, invoice or bank statement.  

NOTE: A complete application must include all of the above-referenced supporting documents. Submitting your application without the proper supporting documents will not only delay processing of your loan, it will also delay our ability to serve other businesses.

Final day to apply for the PPP Loan is Sunday, June 28th.
